
Prostate cancer has emerged as the most frequently diagnosed cancer among men in South Korea. Once regarded as a relatively uncommon malignancy primarily affecting older demographics, it has rapidly transitioned into a major public health threat compromising men's long-term wellness.
The primary danger lies in its silent progression; localized prostate cancer typically presents few, if any, distinct symptoms during its initial stages. By the time noticeable complications surface and patients seek medical intervention, the disease has often already advanced significantly.
As case numbers climb sharply, medical experts emphasize that South Korea needs an urgent national discussion regarding screening protocols to enable early detection. They state it is time for a broader public conversation on integrating the prostate-specific antigen (PSA) test—a simple, highly effective blood test—into the state-funded national health screening program.
To address these growing concerns, the Korean Urologic Oncology Society hosted a press briefing on the 16th at the Korea Press Center in Seoul, releasing its comprehensive "2026 Prostate Cancer Fact Sheet." During the briefing, the society detailed the accelerating incidence rates, explored key metabolic and lifestyle risk factors, identified gaps in diagnostic and therapeutic access, and underscored the necessity of establishing a state-backed early screening infrastructure centered on PSA testing.
"Prostate cancer has already become a critical public health challenge, ranking first in incidence among all male cancers in South Korea, yet a national early-screening system remains entirely absent," said Jung Byung-chang, president of the Korean Urologic Oncology Society and a professor of urology at Samsung Medical Center. "We believe this fact sheet clearly illustrates the true disease burden of prostate cancer alongside the modern realities of diagnosis and treatment. We hope it serves as a catalyst to build public consensus on the vital importance of early screening."
A 2.6-Fold Jump in Ten Years: Beyond Simple Aging
During the presentations, Park Yong-hyun, a professor of urology at Seoul St. Mary’s Hospital, provided an in-depth analysis of South Korea's shifting epidemiological landscape, risk correlations, and evolving therapeutic environments.
In 2023, the number of newly diagnosed prostate cancer patients reached 23,928—a staggering 2.6-fold increase compared to the 11,095 cases recorded in 2014. Prostate cancer now constitutes 15.0% of all male cancer diagnoses nationwide, officially overtaking lung cancer (14.5%) and stomach cancer (12.8%) to claim the top spot.
Crucially, experts note that this surge cannot be attributed solely to an expanding elderly population. The age-standardized incidence rate, which adjusts for demographic shifts, rose by approximately 43%, climbing from 21.1 per 100,000 individuals in 2006 to 30.2 in 2023. This data demonstrates that the absolute disease burden of prostate cancer is compounding independently of demographic aging.
An analysis by age demographic revealed that the spike was most pronounced among individuals in their 70s and those aged 80 and older.
Furthermore, substantial disparities were observed across income levels. The crude incidence rate within the highest-income bracket (the 20th quintile) stood at 191.04, roughly seven times higher than the 27.03 recorded in the 7th quintile. The society indicated that this variance likely highlights unequal access to screening opportunities and voluntary medical care rather than a purely biological difference in actual cancer development.
These regional and socioeconomic imbalances extend into the therapeutic phase as well. Gaps were confirmed in patient access to advanced robotic surgeries, highlighting deep-seated systemic inequities spanning across the entire spectrum of prostate cancer diagnosis and treatment.
Modern lifestyle factors and metabolic conditions have further accelerated these risks. Individuals managing diabetes, hypertension, or dyslipidemia exhibited a heightened vulnerability to prostate cancer, with abdominal obesity and physical inactivity acting as prominent contributing factors. Notably, the incidence rate among long-term smokers with a history spanning 30 years or more was 5.3 times higher than that of early-stage smokers.
"As South Korea transitions into a super-aged society, prostate cancer numbers will inevitably continue their upward trajectory," Park stated. "Given both the mounting volume of patients and the intensifying clinical burden of the disease itself, a proactive national-level response is no longer optional."
Few Early Symptoms: How Routine Blood Tests Enable Early Detection
The presentations repeatedly returned to a foundational clinical reality: early detection saves lives.
Because early-stage prostate cancer remains largely asymptomatic, patients who wait for overt warning signs—such as severe urinary difficulties, hematuria (blood in the urine), or localized bone pain—frequently face a disease that has already progressed significantly. Conversely, therapeutic outcomes are exceptionally high when the malignancy is caught early while still localized within the prostate gland. According to society data, the five-year survival rate for localized prostate cancer exceeds 95%.
The PSA test is the global standard for capturing the disease before it spreads. Requiring only a standard blood draw from the arm to monitor prostate-specific antigen thresholds, the physical and financial strain of testing remains low. Medical bodies widely recommend routine annual or biennial PSA testing for men aged 50 and older, while individuals with a documented family history or other high-risk factors are advised to begin evaluations even earlier.
Despite these verified clinical benefits, prostate cancer screening is currently excluded from South Korea’s state-funded national cancer screening program. Consequently, citizens must independently identify their risk profiles and pay out of pocket for the procedure. This lack of structural support leaves significant blind spots, leaving men unaware of the need or causing them to miss windows for early intervention due to regional isolation or financial constraints.
The society highlighted an extensive body of international clinical data proving that systematic, PSA-based screening significantly reduces advanced symptomatic presentations, metastatic progression, and overall mortality. With the disease now ranking as the primary cancer threat to South Korean men, the society urges immediate policy discussions on introducing early screening tailored to South Korea’s healthcare logistics.
"Patient survival rates and subsequent quality of life diverge drastically based on the timing of the initial diagnosis," noted Lee Seung-hwan, a professor of urology at Severance Hospital. "Rigorous global studies have definitively established that routine PSA screening diminishes the risk of metastatic cancer and lowers disease-specific mortality."
Lee added: "Despite prostate cancer assuming the highest incidence rate among male cancers in South Korea, it remains entirely left out of our national preventive framework. It is imperative that we initiate immediate administrative discussions to implement a state-backed screening protocol, ensuring that every citizen can access timely diagnostics regardless of geographic location or socioeconomic status."
Moving forward, the society plans to leverage the publication of the 2026 Fact Sheet to actively raise public awareness of prostate cancer and continue its systematic efforts aimed at expanding early detection and appropriate treatment protocols.
