In that exact moment, shock sets in and your mind begins to race. Do I really need surgery right away? This is the heavy question facing an increasing number of patients today. While thyroid cancer remains the most frequently diagnosed cancer in South Korea, the medical community is fundamentally rethinking whether every patient requires immediate surgical intervention.
The Dilemma of Early Detection and Overdiagnosis
The vast majority of thyroid cancers are papillary carcinomas, named for the finger-like appearance of their cells under a microscope. Other types—such as follicular, medullary, and anaplastic carcinomas—account for about 15% of cases. Among them, anaplastic carcinoma is exceptionally rare but highly aggressive, requiring entirely separate, immediate treatment.
However, the group driving the current medical shift consists of a specific subset of papillary thyroid cancer: low-risk papillary microcarcinoma. These tumors are tiny, grow incredibly slowly, and carry an exceptionally low risk of metastasis. In fact, autopsy studies show that roughly 10% of people who died from entirely unrelated causes lived their entire lives with undetected papillary thyroid cancer, completely symptom-free.
The core issue driving the rise in diagnoses is technological. As advanced ultrasound imaging has become widespread, clinicians are catching minute nodules that previously would have gone unnoticed. While a 1-centimeter lesion contains roughly one billion cancer cells, its mere presence does not guarantee it will ever spread or cause harm. Studies in both South Korea and the United States reveal that while thyroid cancer diagnoses have soared, the mortality rate has remained virtually unchanged. This discrepancy has led experts to warn against overdiagnosis—detecting small, indolent cancers that would never have caused health problems, yet leading to unnecessary surgeries.
Redefining Standards: What Active Surveillance Means
In response to these findings, global treatment standards are evolving. In 2025, the Korean Thyroid Association issued updated clinical recommendations endorsing "active surveillance" for low-risk papillary thyroid microcarcinoma. The American Thyroid Association and European medical bodies have similarly embraced this strategy to curb overtreatment.
Active surveillance is far from passive neglect; it is a structured, highly monitored protocol. For the first two years, patients undergo ultrasound imaging and thyroid function tests every six months, transitioning to an annual schedule thereafter. If the cancer shows signs of growth or begins to spread, surgery is promptly scheduled.
The criteria for this watchful approach are strict: the tumor must be one centimeter or smaller, with absolutely no evidence of lymph node metastasis, invasion into surrounding tissues, or spread to distant organs like the lungs or bones. For any case falling outside these rigid parameters, surgery remains the gold standard.

Today, the modern consensus in thyroid care emphasizes shared decision-making over immediate, unilateral intervention, allowing doctors and patients to collaboratively choose monitoring when the risk is low.
Considering the Long-Term Impacts of Surgery
The shift toward surveillance is also driven by a realistic look at quality of life after the operating room. Thyroid surgery is typically divided into a hemithyroidectomy (removing half the gland) or a total thyroidectomy (removing the entire gland). If the entire thyroid is removed, patients face a lifelong commitment to daily thyroid hormone replacement medication.
While surgery effectively eliminates the immediate tumor, it is not without lasting consequences. Many patients report experiencing chronic fatigue or voice changes post-surgery. Furthermore, adjusting hormone dosages can be a delicate balancing act, occasionally triggering heart palpitations and weight fluctuations.
Because these side effects can permanently alter a patient's daily life, the medical community continues to debate whether immediate surgery for low-risk patients offers a true survival benefit. Hearing the word "cancer" naturally triggers an instinct to operate. However, modern medicine is proving that a tumor’s size, type, and biological behavior matter far more than its label alone. The clinical norm is changing: a diagnosis of thyroid cancer no longer automatically means a direct path to the operating room.
