Muscle Loss Linked to Rapid Kidney Decline: Mortality Risk Increases 3.8-Fold

A new long-term study has revealed that the loss of muscle mass is a critical, and often fatal, factor for patients suffering from chronic kidney disease (CKD). Researchers confirmed that a decline in muscle mass significantly accelerates the deterioration of kidney function and can increase the risk of death by as much as 3.78 times.

On March 12, in observance of "World Kidney Day," the National Health Research Institute—under the Korea Disease Control and Prevention Agency—released the findings of the 'KNOW-CKD' study. This large-scale research project followed chronic kidney disease patients over an extended period to monitor health outcomes.

Muscle Mass as a Key Clinical Indicator

The study, led by Professor Oh Guk-hwan of Seoul National University Hospital, found that the patient group with the lowest muscle mass faced a risk of kidney function deterioration 4.47 times higher than the group with the highest muscle mass.

The statistical gap is stark. In the group with the highest muscle mass, the rate of kidney function decline was limited to 14.3%. Conversely, in the group with the lowest muscle mass, that figure soared to 42.5%, nearly tripling the rate of decline. These results remained consistent even after researchers adjusted for variables such as age, diabetes, and hypertension, establishing muscle mass itself as a primary indicator of renal health.

Patients with CKD are particularly vulnerable to rapid muscle wasting compared to the general population. This is driven by a complex interplay of chronic inflammation, metabolic abnormalities, and the accumulation of uremic toxins in the bloodstream, making muscle preservation an urgent clinical challenge.

The Danger of Protein-Energy Wasting (PEW)

The research highlights that the risk extends beyond simple muscle loss to a more severe state known as Protein-Energy Wasting (PEW). This condition occurs when the body's overall protein and energy stores are critically depleted.

According to international diagnostic standards, a patient is classified as having PEW if they meet three or more of the following four criteria:

  • Serum Albumin Decrease: Levels below 3.8g/dL.

  • Low Body Mass Index: A BMI below 23.0.

  • Skeletal Muscle Loss: A measurable decrease in muscle mass.

  • Insufficient Protein Intake: Consuming less than 0.6g of protein per kg of body weight daily.

An analysis of ,238 CKD patients not yet on dialysis showed that those meeting three or more of these indicators had a mortality risk 3.78 times higher than those meeting none.

Early Intervention is Critical

Significantly, the study revealed that even moderate muscle and protein loss is dangerous. While previous medical consensus focused on those meeting three or more criteria, this study found for the first time that meeting just two criteria increases the risk of death and cardiovascular disease by 2.78 times.

"This research emphasizes the necessity of proactive management starting from the earliest stages of muscle loss," stated Lim Joo-hyun, head of the Endocrine Kidney Disease Research Division. "Preventing muscle wasting must be recognized as an essential element of treatment to stop the progression of chronic kidney disease."
