Shifting Trends in Stomach Cancer: Why Young Women Face a Stealthier Threat

Data released by South Korea's Central Cancer Registry show that the nation recorded 28,943 new stomach cancer cases among men and women combined, accounting for 10% of all cancer diagnoses and ranking fifth overall. There were 19,295 male patients (ranking third among men) and 9,648 female patients (ranking fifth among women). While stomach cancer long held the number one spot for overall cancer incidence in South Korea, rates have steadily declined as the population reduces its consumption of salty or charred foods and participates in routine screening gastroscopies. However, a troubling challenge remains: the stubbornly low survival rate among female patients under the age of 50.

Tracing the Evasive Nature of Diffuse-Type Tumors

While female stomach cancer patients aged 60 and older exhibit higher survival rates than their male counterparts, the trend completely reverses for patients under 50. A research team led by Professors Kim Na-young and Choi Yong-hoon of the Division of Gastroenterology at Seoul National University Bundang Hospital uncovered this discrepancy after analyzing survival outcomes by sex, age, and tumor histology among 14,839 stomach cancer patients diagnosed between 2003 and 2023.

The researchers attributed the lower survival rates in younger women to distinct tumor biology. Women under 50 were found to have a significantly higher prevalence of "diffuse-type" stomach cancer, a subtype where malignant cells do not form a single, distinct mass but instead scatter infiltratively along the stomach wall. This pattern is particularly striking among women under 40, where diffuse-type tumors account for up to 80% of all cases. Unlike the more common "intestinal-type" stomach cancer, which forms a clear tumor mass that is easily visible during an endoscopy, diffuse-type cancer is exceptionally difficult to detect early and far more challenging to treat. Medical experts note that individuals with established risk factors, such as a family history of gastric cancer or Helicobacter pylori infection, require regular screenings and aggressive preventive care.

Distinguishing Chronic Indigestion From Early Warnings

Stomach cancer is notoriously difficult to diagnose in its early stages because it rarely presents with distinctive symptoms. Mild abdominal discomfort, bloating, subtle pain, loss of appetite, unexplained weight loss, and anemia are frequently mistaken for ordinary indigestion. Because early-stage lesions are often asymptomatic, they are most commonly discovered by chance during routine checkups. Consequently, women in their 20s through 40s who possess hereditary or environmental risk factors should undergo regular gastroscopies, even if they are outside the standard age brackets for national screening programs.

Dietary habits play a foundational role in determining overall risk. According to data from the Korean Gastric Cancer Association, individuals who regularly eat high-sodium foods face a 4.5-times higher risk of developing stomach cancer than those who maintain a low-salt diet. The recent national decline in gastric cancer cases is heavily credited to public health shifts away from salt-preserved foods. Conversely, processed meats like sausages and ham remain classified as Group 1 carcinogens by the World Health Organization's International Agency for Research on Cancer, while smoking continues to cause severe, direct damage to the gastric mucosa.

To proactively lower the risk of stomach cancer, clinicians recommend a consistent diet rich in fresh, whole fruits and allium vegetables—such as garlic, onions, green onions, and chives—which naturally combat cellular oxidation in the stomach lining. Because manufactured dietary supplements have not demonstrated a verifiable cancer-preventive effect, the most reliable protection remains avoiding excessive sodium, minimizing carcinogen exposure, and consuming nutrient-dense foods in their natural form.
