
As sweltering summer weather continues with daytime temperatures hovering around 30°C, the risk of heat-related illnesses like heatstroke and heat exhaustion is climbing sharply. Children, who frequently spend long hours playing outdoors, are particularly vulnerable. While it is easy to dismiss their post-play lethargy, headaches, or dizziness as standard summer fatigue, prolonged exposure to intense sunlight requires heightened vigilance from parents and caregivers.
Compared to adults, children possess less-developed thermoregulatory systems. Their sweat glands are not fully matured, which significantly reduces their physiological capacity to dissipate heat through sweating. Furthermore, young children often fail to recognize or articulate symptoms of dehydration and overheating, meaning their clinical condition can deteriorate with alarming speed.
Caregivers should immediately suspect heatstroke if a child becomes unusually limp, exhibits slurred speech, responds sluggishly after being awakened, or struggles to walk following outdoor activities on a hot day. Other critical warning signs include seizures, repeated vomiting, excessively hot skin, or an unusually pale complexion.
"Heatstroke is a life-threatening medical emergency," warned Han Sang-soo, a professor of emergency medicine at Soonchunhyang University Bucheon Hospital. "If these severe neurological or physical symptoms appear, call emergency services immediately. In the meantime, move the child to a shaded or air-conditioned area and use cold water or cold, wet towels to lower their body temperature as rapidly as possible."
The Critical Window: Preventing Multiple Organ Failure
Medical experts warn that many caregivers inadvertently miss the critical window for effective treatment by adopting a "wait-and-see" approach, assuming the child will recover with basic rest. With heatstroke, the speed at which core body temperature is reduced directly determines the long-term prognosis. Immediate cooling drastically mitigates the risk of irreversible brain damage and multiple organ failure—a catastrophic chain reaction where vital organs, including the brain, heart, kidneys, liver, and lungs, sustain severe damage and cease normal function.
Physiologically, heatstroke is an ultra-emergency medical condition characterized by a core body temperature soaring to 40°C or higher, causing the body's internal thermoregulatory mechanisms to entirely collapse. This failure triggers profound central nervous system abnormalities, ranging from altered consciousness to seizures, alongside widespread systemic damage. In severe cases, the mortality rate can exceed 50%, making rapid recognition and early intervention absolutely paramount.
To prevent these dangerous heat-related conditions, health officials recommend restricting outdoor activities between the peak heat hours of noon and 5 p.m. on days when extreme heat warnings are issued. An analysis from the Korea Disease Control and Prevention Agency’s (KDCA) heat-related illness emergency room surveillance system spanning 2013 to 2022 revealed that roughly 60% of heat illnesses among children aged 6 to 13 occurred in open outdoor spaces, such as school athletic fields or public parks, with nearly half of all cases concentrated between noon and 5 p.m.
Vehicles present an especially severe hazard, as internal temperatures can spike to lethal levels within minutes; children must never be left unattended in a car under any circumstances. If outdoor exposure is unavoidable, children should wear loose, breathable clothing, a wide-brimmed hat, and hydrate frequently. However, a crucial safety rule applies: never force water or fluids down the throat of a child with reduced consciousness, as this can easily cause fluids to enter the airway, resulting in life-threatening aspiration pneumonia or suffocation.
