"They told me my blood pressure was a bit high during pregnancy, but doesn’t it go back to normal after I give birth?"

Many women brush off gestational hypertension, assuming that delivery entirely resolves the issue. However, for those who experienced preeclampsia—formerly known as toxemia of pregnancy—childbirth is often just the beginning of a longer health journey.

According to 2024 birth statistics from Statistics Korea, the average age of South Korean women giving birth to their first child has risen to 33.1, marking the highest age since related data collection began in 1990. Because the risk of preeclampsia escalates with advanced maternal age, the number of patients seeking care for the condition surged by approximately 70% between 2016 and 2020, according to data from the Health Insurance Review & Assessment Service.

The most alarming aspect of this trend is that delivery does not guarantee a clean bill of health. Once immediate postpartum blood pressure levels stabilize, hospitals and patients frequently cease close monitoring, leaving underlying vascular damage to accumulate unaddressed.

Understanding Preeclampsia and the Postpartum Window

May 22 marks World Preeclampsia Day, an initiative co-founded in 2017 by global maternal health organizations, including the Preeclampsia Foundation and the International Society for the Study of Hypertension in Pregnancy. This year’s campaign theme, “Know Her Symptoms,” aims to raise awareness that preeclampsia can develop or manifest up to six weeks after childbirth, underscoring the vital importance of recognizing postpartum warning signs.

As women age, blood vessels naturally stiffen and chronic inflammation accumulates, which can prevent placental blood vessels from properly embedding into the uterine wall. Medically, pregnancy at age 35 or older is classified as advanced maternal age.

The condition's historical name, "toxemia of pregnancy," is a misnomer, as it incorrectly implies that the illness is caused by toxins circulating in the body. In reality, preeclampsia is characterized by a dangerous combination of high blood pressure and protein excreted in the urine (proteinuria) after 20 weeks of gestation. If left unchecked, preeclampsia can progress into eclampsia, a life-threatening condition marked by generalized convulsive seizures or a total loss of consciousness.

The root cause of preeclampsia resides in the placenta. When early placental blood vessels fail to anchor correctly, blood flow becomes unstable, damaging blood vessels throughout the mother’s body and potentially harming the brain, liver, kidneys, and eyes. Because the underlying cause remains tied to the placenta, the most definitive treatment is delivery. While medications can temporarily lower blood pressure, they cannot fix the underlying placental dysfunction. Consequently, while symptoms typically improve once the placenta is delivered, medical interventions have changed very little over the decades.

The Long-Term Toll on Cardiovascular Health

Because blood pressure frequently returns to normal shortly after birth, many mothers assume they are entirely recovered. When they later experience shortness of breath or a racing heart while climbing stairs, few connect these physical symptoms to their recent pregnancy.

Yet, even when clinical numbers appear normal, structural injury to the blood vessels does not instantly vanish. A significant portion of mothers remain hypertensive three months after delivery, and in severe cases, it can take up to two years for blood pressure and proteinuria to return to baseline. Amid the exhausting demands of newborn care, formal follow-up appointments are frequently neglected, allowing silent vascular complications to accumulate.

The long-term consequences are profound. A 2017 meta-analysis led by researchers at the University of Keele in the United Kingdom, which evaluated data from more than 6.5 million women globally, revealed that a history of preeclampsia correlates with a fourfold higher risk of heart failure and a twofold higher risk of stroke. This elevated risk persisted even after adjusting for age, obesity, and diabetes, demonstrating that the metabolic and endothelial imprint left by preeclampsia can endure for decades.

The vascular damage is not limited to the cardiovascular system. A 2018 cohort study tracking 1.17 million Danish women, published by the Statens Serum Institut in the British medical journal BMJ, uncovered a distinct correlation between preeclampsia and a heightened risk of vascular dementia, proving that these damaged pathways extend directly to the brain. Pregnancy places an extraordinary workload on the heart, with plasma volume expanding by up to 50% by mid-pregnancy. Preeclampsia is essentially a warning sign that the vascular system has reached its absolute limit.

Establishing a Lifelong Strategy for Postpartum Care

To combat this blind spot, this year's World Preeclampsia Day campaign centers on the message: “Check, Know, Share.” The initiative encourages women to monitor their blood pressure, interpret the readings accurately, and actively communicate with healthcare providers. The necessity of this campaign is highlighted by a recent U.S. Preeclampsia Foundation survey of 3,000 pregnant women: while 80% were aware of preeclampsia, a mere 8% could accurately identify its full spectrum of symptoms.

Proactive management begins with identifying high-risk factors, which include being 35 or older, a first-time pregnancy, obesity, pre-existing hypertension, diabetes, kidney disease, a family history of the condition, or a multi-fetal pregnancy. Women matching any of these criteria should consult their physicians regarding preventative low-dose aspirin therapy. For those with low dietary calcium, supplementation and regular aerobic exercise can also mitigate risk. Immediate medical evaluation is required if a woman experiences severe headaches, vision changes, upper abdominal pain, or sudden swelling after 20 weeks of pregnancy.

Post-delivery guidelines from the American Heart Association (AHA) and the European Society of Cardiology (ESC) recommend that women who experienced preeclampsia undergo comprehensive checks of their blood pressure, blood sugar, and cholesterol within 6 to 12 months postpartum, followed by annual cardiovascular screenings. Consistently tracking blood pressure at home at least twice a week provides an invaluable record. Crucially, women should disclose their history of preeclampsia to every healthcare provider they see, as this single detail can fundamentally optimize their long-term screening protocols.

Following childbirth, a family's focus naturally shifts entirely to the newborn, often leaving the mother's cardiovascular health in a dangerous blind spot. Families must work together to track blood pressure and protect medical checkup schedules, remembering that a diagnosis of high blood pressure during pregnancy does not simply vanish from an obstetrical chart—the heart remembers.
