
It is common practice to take omega-3 or vitamin E for cardiovascular support while maintaining a daily anticoagulant regimen to prevent blood clots. However, if you notice easier bruising or bleeding gums despite no change in your prescription, a newly added supplement may be the cause.
The U.S. Food and Drug Administration (FDA) warns that supplements can significantly alter a drug’s absorption, metabolism, or excretion. Because medications and supplements often share metabolic enzymes in the liver, one can inadvertently strengthen or weaken the other, turning a "beneficial" addition into a health risk.
Large-scale study highlights interaction risks
A study by a research team at Tehran University of Medical Sciences, published in Aging Clinical and Experimental Research, tracked 21,000 adults over 16 years. The researchers found that 4.8% of participants experienced interactions between medications and supplements. Notably, this risk was 2.8 times higher for those with diabetes and 2.2 times higher for those with hypertension.
The most frequent interaction involved taking vitamin E alongside aspirin. Other significant risks were identified in the combination of magnesium or potassium supplements with blood pressure medications.
Adverse events on the rise
This issue is gaining traction globally. In South Korea, reports of adverse events related to health functional foods tripled in just three years, rising from 1,117 cases in 2022 to 3,551 in 2025. Common nutritional supplements, such as vitamins and minerals, topped the list of reported items.
The core of the problem lies in the distinction between regulated pharmaceuticals and health functional foods. While the Ministry of Food and Drug Safety in Korea and the FDA in the U.S. provide oversight, the biological reality remains: taking these substances together can lead to unexpected and dangerous outcomes.
High-risk combinations: Anticoagulants and blood pressure meds
Patients on the anticoagulant warfarin must be exceptionally cautious. Warfarin works by inhibiting vitamin K; therefore, supplements high in vitamin K can neutralize the drug’s effectiveness. Conversely, vitamin E, omega-3, and Korean red ginseng can inhibit platelet aggregation. When paired with anticoagulants, these can dangerously increase the risk of internal bleeding.
Blood pressure medications also require scrutiny. For example, taking potassium supplements while on potassium-sparing diuretics can lead to hyperkalemia (excessive blood potassium levels), which can be life-threatening.
The danger of "Supplement Overlap"
Vitamin D is a frequent offender in cases of unintentional overdose, as it is often present in multivitamins, calcium supplements, and eye-health products simultaneously. According to the U.S. National Institutes of Health (NIH) and the 2025 Dietary Reference Intakes for Koreans, the upper limit for adults is 4,000 IU per day. Exceeding this can lead to hypercalcemia, kidney impairment, or arrhythmia.
Furthermore, minerals like calcium, iron, magnesium, and zinc often compete for the same absorption pathways. Taking them all at once may result in lower absorption rates than expected, rendering the supplements less effective.
Essential checklist for supplement users
Consult a Professional: If you take anticoagulants or blood pressure medication, speak with a physician or pharmacist before adding omega-3, vitamin E, or Korean red ginseng.
Check for Duplication: Audit your total vitamin D intake across all products to ensure you stay below the 4,000 IU limit.
Time Your Doses: Thyroid medication should be taken on an empty stomach. Separate it from calcium or magnesium by at least 2 hours, and from iron by at least 4 hours.
Pregnancy Precautions: Avoid high doses of animal-based vitamin A (retinol), which is linked to fetal malformations.
Screen for Family: Before gifting supplements to a spouse or parent, confirm their current medication list to avoid accidental interactions.
Supplements are not universally beneficial; their safety depends entirely on how they interact with your existing medical profile.
