The Illusion of Decline: Nearly Half of Older Adults Gain Cognition and Mobility

Do you know someone past 70 who seems sharper than they used to be? Or an octogenarian who still strides briskly around the neighborhood? While it is easy to dismiss these individuals as rare exceptions, a study published in the international journal Geriatrics challenges the long-held assumption that aging is a one-way street toward decline. Tracking participants for up to 12 years, researchers discovered that nearly half of adults aged 65 and older actually improved in either cognitive function or walking speed—gains that have historically been masked by statistical averages.

The study was led by Dr. Becca Levy, a professor of epidemiology and psychology at the Yale School of Public Health and the author of Breaking the Age Code (published in South Korea as The Illusion of Getting Older). Drawing on data from the Health and Retirement Study—a federally supported longitudinal survey conducted by the National Institute on Aging and the University of Michigan—the research team analyzed changes in more than 11,000 older Americans.

The results upended conventional medical narratives. Comparing baseline measurements with final outcomes over a 12-year window, the team found that 45% of participants finished the study with better scores in cognition or mobility than where they began.

Shifting the Focus from Decline to Growth

These upward trajectories have largely gone unnoticed because traditional geriatric research typically categorizes older adults' function into binary outcomes: "decline" or "no decline." When individuals are lumped into a single group average, overall decline dominates the data. By designing an analytical approach capable of tracking individual improvement, Levy’s study brought a massive, resilient segment of the population into view.

The primary variable driving these positive health changes was a participant's mindset. The more positively individuals viewed the aging process at the start of the study, the more likely they were to post gains in both brainpower and walking speed. While the adjusted odds ratios—1.04 for cognitive improvement and 1.09 for walking speed—appear numerically modest, the statistical association remained robust even after accounting for age, sex, chronic illnesses, and depressive symptoms.

This connection between belief and biological function operates along two distinct pathways:

  • The Behavioral Pathway: Individuals with an optimistic outlook on aging maintain higher health expectations, making them fundamentally more likely to exercise, maintain a balanced diet, and proactively seek medical care.

  • The Biological Pathway: Mindset directly regulates stress. A study tracking 30 years of cortisol data revealed that older adults with positive perceptions of aging maintained lower levels of this chronic stress hormone, which is notorious for damaging cognitive and physical health. Remarkably, even among carriers of the high-risk Alzheimer’s gene (APOE ε4), a positive mindset was associated with a 49.8% lower risk of developing dementia.

To reshape how we internalize aging, Levy recommends breaking the everyday habits that reinforce ageism. First, viewers must critically evaluate media representation. Passively consuming media that depicts older individuals as exclusively frail or slow internalizes those very traits as an inevitable script. A parallel South Korean study, authored by Professors Chung Soon-dool of Ewha Womans University and Kim Mi-ri of Korea National University of Transportation and published in Behavioral Sciences, confirmed that exposure to negative media images directly correlates with heightened anxiety and stereotypes surrounding old age.

Second, individuals should stop defaulting to the phrase, "I must be getting old," whenever a minor lapse occurs. Blaming age for a misplaced key or a forgotten name reinforces the illusion of inevitable decay; attributing it to a busy schedule or fatigue reframes the narrative. Finally, keeping active, healthy older adults in one's social circle resets baseline expectations for what later life looks like.

While observational data cannot definitively establish causality, the study highlights a profound societal truth: age bias is a learned behavior, meaning the script for growing older can always be rewritten.
