
As the opening of the 2026 Milan-Cortina d'Ampezzo Winter Olympics approaches in just three days, controversy is once again swirling regarding the participation of transgender athletes in elite competition.
The inclusion of transgender athletes has long been a flashpoint in the sports world. Most disciplines strictly segregate events into male and female categories. However, the gender transition process—which involves various stages such as Gender Reassignment Surgery and Hormone Replacement Therapy (HRT)—creates a demographic that some argue cannot be classified by traditional binary means.
The Debate: Respect vs. Equality
This issue presents a significant dilemma for the Olympics, which prides itself on "respect" as a core value. If participation is determined solely by Biological Sex, the value of inclusivity and respect is compromised. Conversely, broadly allowing transgender participation raises concerns about undermining the value of "competitive equality."
The controversy reached a fever pitch during the 2024 Paris Olympics when Imane Khelif of Algeria won the gold medal in the women’s 66kg boxing final. Khelif had been disqualified by the International Boxing Association (IBA) just a year prior for allegedly possessing XY Chromosomes.
At that time, the International Olympic Committee (IOC) maintained that "gender cannot be decided solely by chromosomes." However, reports indicate that since last November, regulations have tightened to prevent athletes who transitioned after puberty from competing in women's events. Consequently, many in the sports community believe these games may effectively be the final Olympics for transgender athletes under current rules.

Medical Research: "No Significant Advantage"
Recently, research from the medical community has emerged that directly challenges the claims of the sports establishment. A research team from the University of São Paulo in Brazil conducted a Meta-analysis of 52 studies examining athletic performance and strength before and after gender transition. Their findings suggested no significant difference in athletic ability between Transgender Women and Cisgender Women (biological women).
The study found that Transgender Women who received Hormone Therapy for one to three years showed significantly higher body fat percentages compared to biological males, indicating that female physical characteristics had already stabilized. Furthermore, the researchers found no significant differences in Upper and Lower Body Strength, VO2 Max (maximum oxygen consumption), or Muscle Mass when compared to Cisgender Women.
According to this data, there is no definitive medical basis to claim that individuals who were male before transitioning naturally overpower biological women in terms of strength or stamina.
A Contentious Future
The research team published their findings in the prestigious British Medical Journal (BMJ). However, critics argue the data is still insufficient to end the debate, as it does not specifically address the physiological impact of transition on "elite-level athletes," which remains the heart of the Olympic controversy.
Despite the ongoing friction, one transgender athlete is set to compete in these Winter Games. Elis Lundholm from Sweden will participate in the women's Freestyle Skiing Moguls event. While Lundholm’s personal best—an 18th-place finish at last year's World Championships—places her outside of immediate medal contention, her presence is historic. As she may be the last transgender athlete to compete under the current Olympic framework, her performance is expected to draw intense global scrutiny.
