
As summer heat lingers, many people reach for an iced Americano or stick strictly to hot coffee. Problems arise, however, when coffee begins replacing water entirely. While coffee offers notable health benefits, it also acts as a diuretic, and heavy intake can heighten the risk of dehydration. Furthermore, even sugar-free black coffee can increase the risk of hyperlipidemia (dyslipidemia) when consumed in large quantities.
Why Coffee Cannot Replace Water
Caffeine is consumed not only through coffee, but also through tea, soft drinks, and foods such as chocolate. When coffee is regularly paired with these items, total caffeine intake can climb significantly higher than intended. Combining coffee with alcohol—another strong diuretic—further accelerates fluid loss and increases the frequency of urination. Left unchecked, dehydration can deplete essential electrolytes and reduce overall blood volume, potentially leading to medical shock.
Older adults face a heightened risk because the central nervous system's thirst mechanism naturally weakens with age. Because blood is composed primarily of water, severe fluid loss causes it to become more viscous. For individuals with underlying vascular conditions such as hyperlipidemia or arteriosclerosis, this increased blood viscosity raises the likelihood of blood clots. Healthcare experts emphasize that even without feeling thirsty, individuals should drink plenty of plain water alongside caffeinated or alcoholic beverages.
The Link Between Black Coffee and High Cholesterol
Although plain black coffee is widely regarded as a healthy, zero-sugar beverage, excessive consumption can elevate low-density lipoprotein (LDL) "bad" cholesterol levels. While drinking up to three cups a day generally poses little risk, consuming four or more cups warrants caution.
The primary factor is cafestol, a natural plant oil found in coffee beans that accumulates heavily in the layer of foam (crema) atop a fresh cup. Once ingested, cafestol enters the liver and interferes with its natural ability to regulate cholesterol synthesis.
According to the European Society of Cardiology’s 2021 clinical guidelines, drinking nine or more cups of coffee per day can increase the risk of death from atherosclerotic cardiovascular disease—where arteries become blocked by cholesterol buildup—by up to 25%. Conversely, consuming three cups or fewer showed no harmful impact. Notably, both iced Americanos and decaffeinated coffee contain cafestol. However, drip coffee prepared with a paper filter effectively traps these plant oils, preventing them from raising cholesterol levels.
Balancing Antioxidant Benefits with Moderation
Beyond caffeine, coffee contains beneficial antioxidant compounds such as polyphenols and chlorogenic acid, which help reduce cellular oxidative stress. Numerous global studies demonstrate that coffee consumption—both caffeinated and decaffeinated—can lower the risk of liver cancer, reduce liver inflammation, and help prevent fatty liver disease in patients with underlying liver conditions.
These protective effects are optimal at about three cups of black coffee per day, a level at which cafestol remains largely benign. For individuals managing hyperlipidemia or cardiovascular risk factors, switching to paper-filtered drip coffee offers a safer way to enjoy the beverage without compromising cholesterol health. Above all, medical experts reiterate that coffee is not a replacement for water: adequate hydration requires drinking plain water consistently throughout the day.
