
If you find yourself acting out vivid dreams by punching, kicking, or shouting, you may be experiencing REM Sleep Behavior Disorder (RBD). While it might seem like a simple sleep disturbance, medical experts emphasize that RBD requires careful long-term management, as it often serves as a precursor to Parkinson’s disease.
A significant number of individuals diagnosed with RBD eventually develop Parkinson’s or Lewy body dementia years later. "In REM sleep behavior disorder, the brain’s sleep-regulating function weakens, and the normal mechanism that suppresses muscle activity during sleep is released," explains Dr. Kwon Kyung-hyun, Director of Neurology at Seran Hospital. "This dysfunction is closely linked to Parkinson’s, a neurodegenerative disorder caused by the loss of dopamine-producing nerve cells."
The Silent Progression of Dopamine Deficiency
Parkinson’s disease occurs when dopamine levels become insufficient, impairing the brain's ability to regulate physical movement. This deficiency eventually leads to hallmark symptoms such as hand tremors, slowed movement (bradykinesia), muscle rigidity, and postural instability.
The challenge, however, is that by the time these motor symptoms become visible, a substantial portion of the brain's nerve cells may already be damaged. Experts warn that the disease often progresses silently long before a diagnosis is made.
Beyond Movement: The Early Warning Signs
In addition to sleep disturbances, a reduced sense of smell and chronic constipation are recognized as "prodromal" symptoms—signs that appear before the onset of the main disease. If the aroma of coffee suddenly feels faint or if chronic constipation develops without a clear dietary cause, it may indicate a neurological shift. Other subtle warning signs include a reduction in facial expressions (the "Parkinson’s mask") or a noticeably softer, more monotonous voice.
Professor Jung Yu-jin of the Department of Neurology at Kyung Hee University Hospital at Gangdong emphasizes the importance of early detection. "Parkinson’s can present through sleep, smell, and bowel function long before tremors or gait disturbances appear," says Professor Jung. "Rather than dismissing these changes as simple aging, it is vital to consult a specialist for an accurate evaluation."
Managing the Path Forward
While there is currently no cure to stop the progression of Parkinson’s entirely, early detection allows for interventions that can significantly improve quality of life. Consistent medication and lifestyle adjustments are key to slowing the decline of motor functions. Engaging in regular physical activity—such as walking, swimming, and stretching—is highly beneficial for maintaining the balance and flexibility necessary to manage the condition effectively.
